  2. Both you (Part 1) and your veterinarian (Part 2) will need to complete the claim form.
  3. Sign the claim form
  4. Enclose all itemized paid receipts or invoices with the claim form and return to:

    Remember, if your vet is using EVE your claims can be processed instantly - making the deductible and any uncovered portion of the claim the only cost.
